Changes to registration during Covid-19 Pandemic

Due to the current COVID-19 pandemic, patients are asked to register without coming into the practice. To register you will need to complete our online regsitration form

You will also need to compete the Next of Kin / Carers Form to complete your registration. 

 

How To Register

You may register with the practice, provided you live within our practice area.  If patients change address, please check with reception to see if the address falls within our outer boundary. Please complete both the registration forms below.  Have your NHS number ready (if available). 

If you move out of our practice boundary/catchment area, you will need to re-register with another GP. There are no exceptions to this rule.

You MUST complete both forms to be registered with the practice

All patients registering with us must provide proof of residency in the area, and of legal residency in the UK, if they have no previous UK address and GP proof of residency in the area, this could be a Bank statement or Council Tax statement showing your name and current address. Incomplete forms will delay your application

Please note that your registration will not be processed until you have visited the surgery to sign the forms and give address confirmation. All 4 pages must be complete. 

A passport and Home Office documentation will be needed where indicated. Reception staff will be happy to advise.

Please be aware that the Practice does not accept out of area registrations.
