Private Fees
The NHS does not pay for some of the services we provide. This includes a number of medical examinations and certificates such as sick notes, insurance forms, holiday cancellation forms, medical reports, fitness to travel certificates, private prescriptions and some vaccination services.
Forms and Letters
If you would like the doctor to complete a form or provide a letter for you, then please email us at lewccg.g85727-general@nhs.net with details of your request and a member of our medical reports team will get in contact with you. We will advise you of the procedure and the associated charge.
Lewisham Housing Applications - Lewisham Council does not accept recommendations on medical grounds. If you need housing, please complete a self assessment form online here. The council will get in touch with the doctor if need be.
Please Note GPs do not sign such documents as Passport Photographs/Driving license or any form of certification for identity purposes.
